今回は進化した現在の入れ歯について解説いたします。
みなさんは入れ歯と聞いてどんな印象がありますでしょうか？お年寄りのイメージであったり見た目が悪いイメージを思い浮かべる方が多いのではないでしょうか？以前の入れ歯は、みなさんのイメージ通りのものでした。
しかし、歯科医療は日進月歩です。入れ歯に関しても進化し続けています。今では、より快適で見た目の良い入れ歯が一般的に作れるようになりました。当院で扱っているノンクラスプデンチャーについて解説していきます。
進化した入れ歯の名前はノンクラスプデンチャーです。みなさん聞いたことはありますでしょうか？名前の通り金属の引っ掛ける部分がない画期的な入れ歯です。
じゃあどのように装着するかというと、歯茎の色に似せたピンク色のプラスチックを歯にかけて装着する作りとなっています。プラスチックの部分は特殊な材質になっており、力がかかるとしなる性質があります。
上の図のように同じ歯2本分の入れ歯でも大きさがまったく違うのがわかるかと思います。大きさが小さくなるとお口の中の違和感が違います。できるだけ小さい方がより快適に使えることになります。
さらに金属を使わなければ重さも軽くなります。持ってみるとわかるのですが、ノンクラスプデンチャーは本当に軽いです。お口の中に装着した際に重いものだと疲れてしまいますが、軽ければ長時間着けていても気になりません。また、金属がなければ当然見た目が良くなります。歯茎の色にあわせてピンク色になっていますのでさらに審美性が良くなっています。
金属を使っていない分、強度が落ちてしまいます。そのため、間違って踏んでしまったりすると壊れてしまう可能性があります。落としたくらいでは大丈夫です。また、金属を使っていないため、もし歯茎が痩せてしまったりしてゆるくなった場合は調整が難しいため、大きく変わった場合は作り変える必要があります。あとは、保険適用ではないので費用が多くかかります。
このように利点と欠点はありますが、やはり患者さんには人気で、もともと入っていた金属の入れ歯をノンクラスプデンチャーに変える方が増えてきています。さらに、複数入れ歯が入っている方は、一つ変えると他の入れ歯もノンクラスプデンチャーに変えるという方も多いです。入れ歯を入れた状態の写真も見てみてください。
いかがでしょうか？やはり見た目や大きさの面で人気であることがわかるかと思います。
実は、このノンクラスプデンチャーは進化したもうひとつの入れ歯があります。それは、スーパーナチュラルデンチャーといい、見えない部分にのみ金属を使うというハイブリッドな入れ歯があります。これにより欠点であった強度を強化することでできました。つまり弱点の少ない自然な入れ歯ができたのです。
費用はもう少しかかりますが、より良い入れ歯をお探しの患者さんはぜひ検討してみていただければと思います。
このように入れ歯は大きく進化しています。今までは歯を抜いた後はインプラントの治療が一番快適な治療と考えられてきました。しかし、入れ歯の進化により患者さんにとっては、必ずしもインプラントが最良な治療とは言えなくなってきたのです。高齢の患者さんにとっては、インプラントの手術は体に大きな負担となることがあります。そのような患者さんにぜひオススメしたい入れ歯です。
また、入れ歯はまだまだ進化しています。今後もしかしたら、インプラントよりも快適な入れ歯が開発されるかもしれません。

今回はインプラント治療に関して詳しく解説していきたいと思います。

インプラントに関しては、新聞やテレビなどでかなり有名になり、多くの方が聞いたことがあるかと思います。しかし、当院の患者さんから内容を尋ねてみると実はよく理解していなかったり、間違った情報を鵜呑みにしてしまっていたりすることが多々あります。インプラント＝差し歯と思っている方もいました。そこで、今回インプラントに関して、良いところも悪いところも含めて詳しく解説していきたいと思います。

However, when we ask questions from patients in our dental clinic, we often don’t understand well, or often misunderstand the wrong information. Some people thought that implants = dentures. Therefore, I would like to explain in detail about the implants, including the pros and cons.
右の図でご説明いたします。まず、インプラントは3つの部分に分かれています。I will explain it on the right. First, the implant is divided into three parts.
①フィクスチャー（インプラント本体）Fixture (implant body)
②アバットメント（インプラントの土台）Abutment (implant base)
③上部構造（インプラントの被せ物）Superstructure (implant covering)
フィクスチャーとはインプラント本体の部分を言います。つまり、これがインプラントです。上に見えている被せ物ではなく、骨の中に入っているのが本体なのです。これが意外に知られていないです。そのため、一番重要な部分は骨の中に隠れています。フィクスチャーは、手術で骨に穴をあけて中に埋め込みます。このフィクスチャーは約3ヶ月～6ヶ月程度で骨とくっつきます。そうなったら上に被せ物を作っていく形になります。このフィクスチャーがうまく骨とくっつかないと手術がうまくいかなかったこととなります。

アバットメントは、インプラント体に立てる土台のことです。土台がないと被せ物を入れられません。この土台がインプラント体と被せ物の連結部となります。この土台は形や材質が様々あります。一番適切なものを選択し、セットします。

上部構造は、インプラントの被せ物になります。ここの部分が一番お口の中で見える部分となり、これをインプラントと勘違いしている人が多いです。この部分はあくまで被せ物の部分なので、もし割れたり破損したとしても交換することができます。上部構造も形や材質が様々なため、最も適したものを選択し、セットすることとなります。

このようにインプラントには利点と欠点があります。また、骨の厚みや密度によってはどうしてもできない場合があります。そのため、もし骨がしっかり残っている方にとってはとてもオススメな治療方法となります。インプラントはメンテンナンスが非常に重要となります。そのため、歯医者さんに定期的にメンテナンスに行くことが前提となりますので、しっかりと定期検診に行く習慣をつけておくことが重要です。他にも入れ歯やブリッジ（隣の歯を削って被せ物をつなげ橋渡しを作る治療）などと比べると他の歯に影響を与えず快適に食事ができる治療法です。

この方法は、3種類の薬をまぜて虫歯菌を殺菌する方法です。これは、当院でも行っている治療法です。ただし、この治療法は手技と薬の管理が重要になります。もし、正確に手技や保管が行われなければまったく効果が得られません。そのため、しっかりとした技術習得と徹底した管理が必要となります。今まで虫歯の治療は、虫歯の細菌を感染した歯ごと削って取ってしまうという方法でした。ただ、その場合は、どうしても細菌は目に見えないものなので取り残してしまい再発してしまうリスクがありました。そこで内科的は発想と同様で、薬で直接殺菌するという方法が出てきたのが3mix-MPの基礎と言われています。

オゾン治療においては、確かに歯を削らないで虫歯の細菌を殺菌することができます。ただ、殺菌後に穴のあいた部分の修復が必要になります。通常、白いプラスチックにて封鎖することになりますが、虫歯の細菌に感染した歯質は、やわらかく脱灰しており、その修復するために使うプラスチックの素材がうまくくっつかないことが多いです。そのため、感染した歯質をある程度削って修復することになりますので、まったく削らないで治療することは難しいです。

次に、3mix-MP法に関しましても、虫歯の細菌を殺菌するために3種類の抗生剤を混ぜたものを虫歯の歯の穴に入れて封鎖します。この際に封鎖が完璧でなければ効果がありません。そのため、前述と同様で虫歯の細菌に感染した部分をある程度削って新鮮な歯質を出してからプラスチックの詰め物をすることになりますので、やはりまったく削らない治療というわけではありません。

このようにこれらの治療は通常の虫歯治療に比べると歯を削る量は少ないと言えますが、まったくゼロではありません。虫歯を削る音や振動が苦手という方の問い合わせが非常に多いため、これらの内容をご説明するとがっかりされる方もいらっしゃいます。ただ、それでもかなり快適に治療ができることは間違いありません。ただし、治療方法が完璧でないと効果を発揮できずうまくいかない場合もありますので、その点はご了承していただいた上で行うことがよいかと思います。
